Our team, consisting of an Occupational Therapist, Physiatrist and Rehabilitation Technology Supplier * has specialized training to conduct a comprehensive assessment of your individual seating and mobility needs, while considering your unique abilities, medical issues, lifestyle, home, and work environments. You and your physician, case manager, community therapist, family member, and/or caregiver are also important members of the team. *or vendor of your choice Proper fitting starts with a comprehensive evaluationIf you rely on a wheelchair for your mobility, we can assist you in improving your posture, comfort, and functional independence. Our evaluation often begins with a thorough home assessment prior to the clinic visit, where one of our licensed therapists will assess activities of daily living skills, range of motion, strength, functional mobility, coordination, postural alignment, sensation, and balance as they relate to wheelchair seating and mobility needs. Recommendations may range from modifications or repairs to your existing equipment to prescription for a new manual wheelchair, scooter, or power wheelchair with or without specialized seating. |
After the clinicIf you meet your insurance provider’s qualifications for reimbursement, a prescription for the equipment will be obtained and a letter of medical necessity will be written by a licensed therapist, based on the information gained by your team during your assessment. The equipment will be ordered by the vendor, who will provide future maintenance and repair services for the wheelchair. Delivery and follow-up appointments may be scheduled at the clinic or your home to ensure your satisfaction, as well as proper fit and performance of your new equipment. You and your family member/caregivers will be trained in the proper care and safe use of your new equipment. Referral processSimply have your physician fax a referral for an Occupational Therapy Evaluation and Treatment and Physiatry Consult for Wheelchair Seating and Positioning to 883-1224. Then call us at 883-1211 to schedule an initial evaluation. We can often schedule your first appointment within two weeks of receiving your referral information. |
